In short

Take this to any assessment provider you are considering, including us. It covers what to ask before you book, what to gather, what the clinician may ask you, and what to bring on the day.

One page. You do not need every item. Assessment processes vary, so follow the clinic’s instructions and bring what you have.

Download the checklist (PDF)

What’s on the checklist?

  • Before you book: six questions to ask the provider, including whether the written diagnosis will meet Ontario Autism Program (OAP) requirements and what the fee includes.
  • Gather what you can: reports, school or daycare notes, medical history, and examples of what you have noticed.
  • Questions the clinician may ask: so you can think of real examples ahead of time.
  • On the day: what to bring, what to tell the clinician, and a reminder that you can ask questions.
  • OAP reminder: what the written diagnosis needs to include if you plan to register.

General information only. This is not a diagnostic tool.
