In short

A tracker for writing down what happened during a meltdown, once everyone is calm. Over time, your notes show patterns: what tends to come first, what helps and what makes things harder.

Two pages: a full notes sheet for one meltdown, and a quick log with one row per meltdown for busy weeks.

How do I use it?

  1. Wait until your child is calm and everyone is safe. In the moment, focus on safety, not the notes.
  2. Write down what was happening before, any early signs you noticed, and what the meltdown looked like.
  3. Note how long it lasted, what helped and what made it harder.
  4. On busy weeks, use the quick log on page two instead.
  5. After a few weeks, read your notes back and look for what repeats.
  6. Share your notes with your child’s Clinical Supervisor or any clinician you are working with.
